About the Partnership

Through collaboration and co-design, the Partnership supports sustainable solutions to advance Community Care Hubs (CCH) as a preferred organized delivery system to achieve value-based, community-focused, person-centered health and social care ecosystems. Key Challenges Identified for Community-Based Organizations (CBOs) and Community Care Hubs (CCHs) Serving Rural Communities

Relationships & Collaboration:

The Partnership’s multi-sector stakeholders drive the collaborative’s activities to develop resources and educational opportunities to advance health and social care alignment. The Partnership serves as a unique and neutral forum for representatives and leaders from across health and social sectors to share their individual expertise. For more information about the Partnership, opportunities to support this work, or to get engaged join our mailing list or reach out to info@partnership2asc.org.

Peer Learning, Resources, and Technical Assistances:

The Partnership’s Workgroups are essential to achieving the overall goals of co-designing a plan to align social care and health care. Through the collaboration of Partnership members and subject matter experts, Workgroups are focused on specific issues central to achieving this goal. Additionally, a number of Partnership stakeholders are available to offer technical assistance in communities to organizations considering developing or joining a Community Care Hub. The Partnership also hosts a monthly peer learning series, Coffee and Contracts: A Social Needs Community of Practice.

Revenue, Governance, and Infrastructure:

Partnership stakeholders have developed–and are developing–numerous resources to inform CCH development and maturation. Additionally, the Partnership continues to expand resource offerings to support various business models, revenue streams, and contracting efforts with health care payers, providers, and systems. In Summer 2025, the Partnership and USAging will also release a series of resources and templates to enhance capacity for CCH governance infrastructure in various communities nationwide.

Business Acumen, Value Proposition:

In addition to its Workgroups, the Partnership works closely with USAging and its Aging and Disability Business Institute and the Center of Excellence to Align Health and Social Care to inform, share, and improve access to resources, case studies, and templates to develop business acumen and advance the value proposition for CCH and CBO partners working with health care entities.

CBO Culture Change:

Cross-sector collaboration and co-design is an essential component of fostering the culture change necessary to advance social care delivery reform while ensuring that the vital role of community-based organizations and their service delivery expertise is protected, appreciated, and enhanced.
